Checklist and Forms

In order to fully secure your place in the program, you must complete the following steps:

  1. Submit Your Travel/Medical Consent Form
    You must submit two notarized copies of this form: files/ce/forms/HSP/HSP_BRCE_TravelMedical.pdf. The Spanish government requires this in order for you to travel as a minor unaccompanied by a parent. Please send all requested pages to the Office of Admissions. Include your full name and C00# on all pages.

  2. Activate Your UNI
    Your University Network Identification (UNI) is the identifier by which you can access computer and electronic resources at Columbia. You should activate your UNI as soon as you receive an activation link that will be sent to you by e-mail about a week after you submit your deposit.  
     
  3. Submit Two Photocopies of Your Passport
    For our records, we must receive two copies of the page in your passport that contains your personal data and photograph. We must receive these copies as early as possible in order to reserve your spot in the program. They can be mailed to the same address as the Travel/Medical Consent Form. If you are a non-US citizen and need a visa to travel to Spain, please also send two copies of the passport page showing the visa.
     
  4. Submit Final Tuition Payments to Student Financial Services
    In early June, Student Financial Services will send a complete invoice to your Columbia University e-mail address detailing the tuition and fees. You will be unable to make a payment other than the required $500 deposit until you receive this notice. We will be providing additional information regarding billing and payment options in the next few weeks.

Completing the tasks above will ensure that you are fully prepared to attend the program.
