In order to fully secure your place in the program, you must complete the following steps:
1. Submit Your Travel/Medical Consent Form
You must submit two notarized copies of this form:

This is a requirement of the Jordanian government in order for you to travel as a minor unaccompanied by a parent. Please send all requested pages to the Office of Admissions. Include your full name and C00# on all pages.

2. Submit Two Photocopies of Your Passport and Visa
For our records, we must receive two copies of the page in your passport that contains your personal data and photograph and two copies of the page showing your visa for entry into Jordan. We must receive these copies as early as possible in order to reserve your spot in the program. They can be mailed to the same address as the Travel/Medical Consent Form.
3. Obtain a Visa for Entry into Jordan
For citizens of the United States and Western Europe, we have provided a visa application, a letter from Columbia University explaining your participation in the program, and instructions for obtaining a visa from the Jordanian Consul. Citizens of all other countries must obtain information and visa forms from the Consul of Jordan's website: http://www.jordanembassyus.org/new/consular/visas.shtml.
You should send your visa application to the Consulate along with the letter from Columbia University. The average turnaround time for obtaining a visa from the Washington DC consulate is five business days. Jordan does not maintain an embassy in New York City, so you will not be able to obtain a visa in person once you arrive at Columbia for the program.
4. Submit Your Measles, Mumps, and Rubella Records
Columbia University requires that all students be up-to-date on their measles, mumps, and rubella vaccinations. Your family physician must complete this form for you. The form can be found here: http://ce.columbia.edu/files/ce/forms/HSP/09_10_MMR_Form.pdf.

While you are at your doctor's office getting this form filled out, we recommend that you make sure the rest of your routine immunizations are up-to-date for travel to Jordan. Jordan does not require any special inoculations for entry, but students and their parents should consult with their family physician or a physician at a travel health clinic at least four to six weeks prior to leaving for Jordan to discuss recommendations for preventative vaccinations. Adhering to this time frame will allow any administered vaccinations the time necessary to take effect. Students traveling from certain regions of Africa into Jordan will need to have documentation of Yellow Fever inoculation. 5. Activate Your UNI
Your University Network Identification (UNI) is the identifier by which you can access computer and electronic resources at Columbia. You should activate your UNI as soon as you receive an activation link that will be sent to you by e-mail about a week after you submit your deposit.
Activation of your UNI allows you access to your Columbia University e-mail account. Upon activation, you can log into your e-mail account at cubmail.columbia.edu.
6. Request Your Columbia University ID Card
The University ID card grants you access to the libraries, the student activities center, the physical fitness center, and various other campus resources. In order to obtain a University ID card, please submit a JPEG photo online to http://www.columbia.edu/cu/id/. The photo deadline is June 7. You will need your UNI to log into the secure site.
The color photo must be passport-style: facing forward with your head and shoulders clearly visible. No photos with sunglasses or hats will be accepted.
Students who have previously attended the Summer Programs for High School Students will need to obtain new ID cards.
If you have been admitted to the program after the photo deadline, do not use the online system to submit your photo. You will obtain an ID card upon arrival to campus by taking a photo in person at the ID Center.
7. Submit Final Tuition Payments to Student Financial Services
In early June, Student Financial Services will send a complete invoice to your Columbia University e-mail address detailing the tuition and fees. You will be unable to make a payment other than the required $500 deposit until you receive this invoice. We will be providing additional information regarding billing and payment options in the next few weeks.
Completing the tasks above will ensure that you are fully prepared to attend the program.
